How to obtain and submit a new application form?

An approved testing application is required to schedule your exams.

An application form needs to be submitted to enroll/update your eligibility route for your testing/certification, and additionally, the application form may also require training program details, accommodations, military information, and other declarations if applicable. 

To obtain the new application, you must create and log in to the Credentia Platform. Ensure you access the signup/login page from your state-specific page. 

For example: If you want to enroll in the California - northern region. On the Credentia website select the “find your exam” option and choose your state option provided to navigate to the state-specific page. 

On the state-specific page of Credentia, access the “Credentia Platform” to create or log in to the account of the selected state. See: “How to sign up?” to know more.


The following are the steps to submit the new application form in the Credentia Platform:

Step 1: Log in to the Credentia Platform.

Visit the credentia.com website, navigate to the login page provided, and enter your registered email address and password to log in. 

Step 2: Select Start New Application Form

Click on the “Start New Application” button provided on the dashboard or on the application tab to begin.

Step 3: Choose the eligibility route

First, you must choose your eligibility route from the available options. You can click on each eligibility route option to read its details and make the right selection.

Step 4: Confirm by checking the box

Check the box provided in the eligibility route details section to confirm that you have read the instructions for the selected eligibility route.

Step 5: Select Start to get the application form

Click on the start button to get the relevant testing application form for the selected eligibility route.

Step 6: Complete the form

The application form comprises several sections that need to be completed, and the sections you see may vary depending on the selected eligibility route. 

Note:

  1. See eligibility route details or your state’s candidate handbook for more info. 
    1. Go to Credentia.com and navigate to the state-specific page to access its handbook. 
    2. Navigate to Credentia.com > click on Find My Exam > select the state from the list > scroll down to the Resources section > click on Candidate Handbook.
  2. Ensure that all mandatory sections are completed before submitting your application. 
  3. The application form needs to be submitted to enroll/update your eligibility route for your nurse aide testing/certification, and additionally, the application form may also require training program details, accommodations, military information, and other declarations if applicable.
  4. If you require testing accommodation, please provide evidence of your condition in the designated section. 

Step 7: Submit the form

Once you have completed the application click on the “Submit” button to send the application for review.

 

The submitted application will go through an application review process and post the review. You will be notified by email, you may also log in to the Credentia Platform to view its status. See “How to view application status?”.
